Transition (programme) management
The transition programme is what is involved in making and managing the changes from the current patterns of care provision, to new – including the estate and infrastructure needs and responses. Key inputs will be the SSDP (Strategic Service Development Plan) and the CIAMS (Commissioners Investment and Asset Management Strategy). A transition management programme typically takes each of the key elements of the strategies and expresses them in the context of time (i.e. phases, milestone dates, logic / dependencies etc.), the principal resources required, and essential governance and control arrangements (approval points, Gateways etc.). Outputs from a transition programme normally include bar-charts or tables showing the relationships between key projects or elements, phasing diagrams, cost plans and cash flow forecasts and a risk management plan.

Each step of the process should take place in sequence, e.g. jumping too quickly to ‘Executing’ change without addressing the preceding tasks will almost inevitably result in significant problems.
